Kingston Vacuum Works manufactures and sells Vacuum Forming machines and supplies.  Aircraft Resource Center has a product review available of the Kingston Vacuum Works Mighty Mini Globe-O-Former which includes a few images of this device.

I got this back in....1969(?) for less than ten dollars!

Yep, as Jeffry knows, those old Mattel machines are great.  I built my own vac-forming plate similar to what Kingston offers but for $10 worth of found parts.
I have a full-blown vacforming heater base kit (might have even come from Kingston) but it's very complex and fiddly.  Easier just to use an old toaster oven for small styrene heating and I'm planning on getting some old, second-hand stove for doing full fuselage pieces someday.

Someday.
